Thanks for sharing your present medical situation with the community. You may be aware we cannot provide medical advice or diagnostics over the internet (for your own safety), but your concern certainly warrants a reply.
Is there anything you can think of that has changed (in your life/lifestyle), that might account for the difference in your capabilities from 3 years ago to the present time?
You have shared that you have both COPD and pulmonary hypertension. Both of these diseases can cause shortness of breath (SOB). You've also shared you are using supplemental oxygen therapy. Do you find the oxygen helps with your breathing?
Are you being followed by a physician? Is the doctor also aware of your mental fatigue? Do you think it would be beneficial for you to discuss (the concerns you've shared here), with your private medical doctor?
